According to OSH, the following processes should be followed:
1. Interview witnesses as soon as possible
2. If Possible – Document the accident scene before changes are made
- Take photos
- Draw scaled sketches
- Record measurements
3. Collect support documents
4. Keep all notes and remarks in a bound notebook or three ring binder
5. Record:
- Pre-accident conditions
- Accident sequence
- Post-accident conditions
6. Document victim location, witnesses, machinery, energy sources and other contributing factors
